If you would like us to conduct a free evaluation of your digital camera flash card, please print this form and include it with your flash card. Our mailing address is on the form.

Please include a check or money order, made to Oscar Medina or San Diego Photos and Prints, in the amount of $6 for return postage. I use United States Postal Service Priority Mail with delivery confirmation to mail back your flash card. Priority maile allows us to confirm delivery back to your address. If we are unable to recover your photographs, the only thing you pay is return postage.
